"Unsaid words"

That look was for her. She knew about his plan. She wanted to tell him to be careful. He turned around to tell her that he was worried, indeed; but everything was going to be fine. No words needed.

This whole scene is really deep. Something must’ve happened inside the ambulance, something intimate and mysterious. The way they both look at each other is quite telling. It’s like he’s reassuring her that, “Don’t worry Molly, I’ll be back.” 

Before this scene, Sherlock specifically tells Molly that he’s worried about her because she seems stressed. Sherlock seldom worries. If he does, he never verbalizes it. But with Molly, he says it out loud.

It breaks my heart because I’m sure Molly is in anguish and Sherlock knows about it, but he doesn’t show anything, even when he knows that she’s worried about him. Why?

Emotional context. Sentiment is a chemical defect found in the losing side.

She’s his weakness. He cares about her. Doesn’t want any harm to befall her. He doesn’t want her to get hurt as he knows full well how dangerous Smith is.

People may say that he’s looking at his best friend, John Watson. I disagree. There is no need for Sherlock to turn around and look at John. Sherlock trusts John, he knows that John is going to follow him to the ends of the earth.

Sherlock is definitely looking at Molly. He’s looking at her like he’s afraid for his own life, for humanity, for her. Smith is the embodiment of evil. Sherlock knows, but nobody believes him.

But Molly can see through him and Sherlock knows this fact, so he’s sending her a message through the visual sense: “I’m helpless right now. You can see me. You do count. Please believe me, Molly. I need help.” 

But on the other hand, it’s like his eyes are begging (the despair and desperation is evident in them), they are trying to convey something to her like: “Do not worry about me, you are already under a lot of stress. I promise you, I’ll be fine. I won’t die.” 

It’s a huge dilemma!

As he leaves, Molly looks helplessly at him. She feels restless. It’s like she wants to say something to him, but one look from him silences her. It’s like she’s keeping a secret. It’s like she’s saying, “Goodbye”. She’s worried about him, she LOVES him. And he bloody knows! But Sherlock isn’t manipulating Molly in this scene.

It’s a mutual talk!

This scene on a whole feels so INTIMATE! They are conversing with their eyes. It’s mind-blowing on a whole new level. Don’t believe me, watch the scene again.

Great observations! Particularly the one about why that look is directed at Molly more so than at John. It’s like Sherlock and Molly have their own unspoken language.
